Chapter 4 - The Mandatory Hold

Within fifteen minutes, the quiet atmosphere of the emergency department exploded into controlled, highly structured activity.

Caleb had pressed the emergency code button on the radiology wall console. Three hospital security guards and Dr. Evelyn Chen, the attending emergency medicine physician, arrived at the radiology suite door within sixty seconds.

Julian had tried to smooth over the situation immediately, adopting a posture of injured dignity as he straightened his suit.

"Dr. Chen, thank God," Julian said smoothly, stepping away from Caleb and holding his hands out in a gesture of open reasonableness. "My name is Julian Mercer. My family’s foundation donated the new pediatric wing to this facility last year. My wife, Clara, had a tragic fall at our home tonight. Her brother, who happens to be your technician, has launched an unprovoked verbal assault against me due to an old family feud."

Dr. Chen—a sharp, middle-aged woman with dark hair tied in a neat bun—didn't look impressed by Julian’s name or his foundation. She looked past him, her sharp gaze sweeping over my swollen wrist, my pale, tear-streaked face, and the terrified way I was clutching my stomach.

"Mr. Mercer," Dr. Chen said evenly, stepping into the room. "In this department, donor wall plaques don't diagnose fractures. Scans do. Step outside into the hallway now. Security will escort you to the family waiting room."

"I am not leaving my wife’s side!" Julian demanded, his voice rising in anger. "She is eight months pregnant with my daughter! She is hysterical and unstable!"

"If you refuse to move to the waiting area, Mr. Mercer, security will escort you off the hospital premises entirely," Dr. Chen stated, her voice dropping into an authoritative tone that left no room for negotiation. "This is a medical treatment area, not a corporate boardroom. Officers, escort him out."

The two heavy-set security officers stepped forward, placing themselves between Julian and my bed. Julian glared at Caleb, then at me, his eyes dark with promise of retaliation.

"This isn't over, Clara," Julian spat out in a low whisper before turning on his heel. "We are going home tonight, one way or another."

The heavy door closed, leaving Dr. Chen, Caleb, and me alone in the suite.

Dr. Chen immediately walked over to my side, gently taking my uninjured hand while examining the X-ray images Caleb had loaded onto the main viewing station. She studied the spiral fracture on the screen for several long moments, her expression growing increasingly somber.

"Caleb is right," Dr. Chen said softly, looking at me with deep sympathy. "This is an acute rotational force injury. It is entirely inconsistent with a stumble on stairs. Clara... I am filing a mandatory adult protective services report and placing you under an administrative medical hold."

A wave of relief mixed with sudden, overwhelming terror washed over me. "Can he take my baby? He told me his family lawyers could bribe a judge... he said he’d prove I was mentally unstable..."

"He’s using fear to keep you quiet, Clara," Dr. Chen reassured me gently, pulling a chair over to sit beside my bed. "Under an administrative hold, no one—not even a legal spouse—can remove you from this hospital against your will or without my medical discharge signature. Furthermore, because of the physical trauma and the stress on your body at thirty-four weeks gestation, I am admitting you directly to the High-Risk Labor and Delivery Unit under a Jane Doe alias."

"A Jane Doe alias?" I asked, wiping my cheeks with my sleeve.

"It means your name disappears from the hospital’s public directory," Caleb explained, coming over to stand beside my chair and placing his arm around my shoulders. "Julian can call every hospital administrator in the state, but according to our central computer system, Clara Mercer isn't in this building."

"What about my arm?" I asked, wincing as a sharp throbbing pain shot up to my elbow.

"We need to perform a closed reduction under light sedation to align the bones before putting on a plaster cast," Dr. Chen said. "And we need to perform a continuous fetal non-stress test to ensure the baby hasn't suffered placental distress from the physical shock."

She looked at Caleb. "Caleb, you're off the clock for the rest of the night. Go with your sister."
