Chapter 3 - The Crash

In the sterile theatre of Delivery Room 4, time compressed into frantic seconds. As the anesthesia swept me into the abyss, the world became a symphony of metallic clicks, harsh orders, and the visceral smell of antiseptic iodine.
Dr. Chen did not bother with a delicate Pfannenstiel incision. The baby's profound bradycardia allowed for no aesthetic considerations. With a single, decisive stroke of the scalpel, she made a vertical midline incision from below the navel down to the pubic bone, cutting through subcutaneous tissue and fascia in seconds.
"Suction!" Dr. Chen shouted, her gloved hands parting the tissue. "Retractors! Get that suction in here, now!"
Adrian stood frozen three feet inside the double doors, barred from approaching by the towering frame of Chief Vance and a hospital security guard. His scrub cap was askew, his expensive Italian leather shoes spattered with betadine from the rushed prep.
"Vance, this is an administrative overreach!" Adrian bellowed, though his voice cracked at the edges. "I am the head of this department! Claire is my wife! You cannot perform an unauthorized operative procedure without my signature!"
"Shut your mouth, Adrian," Dr. Vance said, not even turning his head to look at him. His voice was low, carrying the weight of four decades of surgical supremacy. "Your wife signed an advanced medical directive five years ago naming Rebecca Sloan as her surrogate in the event of an ethical impasse with you. It was uploaded to the state compliance repository this morning. You are not her doctor. You are not her proxy. Right now, you are merely a liability in a clean pair of scrubs."
"She was progressing!" Adrian insisted, though his eyes darted nervously to the monitor screens that lined the wall. "Her pelvimetry showed borderline clearance—"
"Borderline?!" Dr. Chen’s voice pierced the air as she incised the lower uterine segment. A massive rush of amniotic fluid and dark, clotted blood surged over the drapes. "The baby is in a complete transverse arrest! The shoulder is impacted behind the symphysis pubis! Look at this lower segment, Marcus—it's stretched thin as paper! Another twenty minutes of labor and she would have suffered a complete uterine rupture and bled out into her abdominal cavity!"
Natalie Brooks stood pressed against the resuscitation cart, her hands clasped over her mouth, her eyes wild with terror. "I... I checked the station... Dr. Whitmore told me it was plus one..."
"You didn't check anything, Brooks!" Elena Martinez snapped, handing a curved clamp to Dr. Chen. "You never performed a sterile digital exam! You logged whatever numbers Dr. Whitmore told you to write down so he could justify keeping her in labor!"
"Elena, you're insubordinate!" Adrian stepped forward, his face flushed purple, but the security officer placed a heavy hand on his shoulder, pinning him in place.
"He's stuck," Dr. Chen said, her tone suddenly dropping into that chilling, quiet register surgeons use when confronting a fatal complication. "The head is deeply wedged. I can't deliver the anterior shoulder. Push from below! Elena, apply suprapubic pressure! Not fundal—suprapubic! Move!"
Elena threw her entire weight onto the lower part of my pelvic bone, trying to dislodge the trapped shoulder of a child who had grown too large for the birth canal through weeks of poorly managed gestational monitoring.
"Fetal heart tone is non-detectable," the anesthesiologist called out from the head of the bed, his voice flat and clinical. "Maternal blood pressure is dropping: eighty over forty. She's bleeding heavily from the uterine angles."
Adrian’s breath caught in his throat. For eighteen hours, he had viewed this room as a stage for his dominance—a place to demonstrate to his proud, unyielding wife that within these walls, his word was absolute law. He had convinced himself that the baby was fine, that Claire was merely exaggerating her distress out of malice toward Natalie, that his medical brilliance could bend biological reality to his will.
Now, looking down at the wide-open surgical field, he saw the truth: a pool of bright arterial blood expanding over the pale blue sheets, and the limp, lifeless arm of an infant protruding from the uterine incision, grey and motionless.
"Get the vacuum!" Adrian suddenly cried out, his clinical composure completely shattering. He lunged forward, breaking past the security guard’s grip. "Chen, let me in! Let me maneuver him! I can do a Zavanelli maneuver—I can push him back in—"
"Don't you dare touch her!" Dr. Chen roared, shielding the field with her forearm. "Get him out of here! He's contaminating the sterile field!"
Dr. Vance grabbed Adrian by the collar of his scrub top, dragging him backward with astonishing force. "You have done enough, Adrian! Step back, or I will have you taken to a holding cell in zip-ties!"
With a final, desperate traction maneuver, Dr. Chen slipped two fingers behind the infant’s clavicle, rotating the torso forty-five degrees. With a heavy, wet sound, the baby was freed from the womb.
The room fell instantly, terrifyingly silent.
There was no cry. There was no gasp for air.
The child Dr. Chen lifted onto the sterile drape was enormous, his skin a mottled, cyanotic blue, his limbs hanging limp like wet cloth. A thick, dark meconium coated his face and shoulders.
"Time of birth: 14:32," Dr. Chen called out, her voice tight with strain. "Apgar is zero. Neonatal team, he's yours. We need to control this hemorrhage!"
The pediatric resuscitation team swarmed the warmer. The infant was transferred immediately beneath the heat lamps. A tiny bag-valve mask was sealed over his face.
*Puff... puff... puff...*
"No pulse," the neonatologist announced grimly. "Starting chest compressions. One, two, three, breathe. One, two, three, breathe. Draw up 0.1 of epinephrine for endotracheal administration. Prepare the laryngoscope!"
Adrian stood against the tiled wall, his arms trembling violently, his eyes fixed on the tiny, unmoving chest being compressed by two thumbs. His chest heaved as if he were the one suffocating.
"He's not breathing," Adrian whispered, the realization finally penetrating his skull. "He's... he's ten pounds... he's not breathing..."

"That's your son, Adrian," Rebecca Sloan said, her voice dripping with ice as she stood beside the recording device. "Take a good look at your work."
